State Licensing Compliance Surveys:
The Office of Medical Facilities Licensing conducts initial licensing and annual
compliance surveys.
Federal Certification and Recertification Surveys:
Under a contract between the Arizona Department of Health Services (ADHS) and the
Centers for Medicare and Medicaid Services (CMS), the Office of Medical Facilities
Licensing conducts initial certification and recertification surveys for non long term care
providers and suppliers participating in the Medicare and Medicaid programs. These surveys
are conducted according to the schedule outlined in the CMS annual contract plan.
Complaint Investigations:
Complaint investigations are conducted for a complaint recieved from the public,
patients, facilities, other agencies, or interested parties that allege violations of
the State licensing rules found in the
Arizona Administrative Code R9-10 or Conditions of Participation requirements found
in the
Centers for Medicare and Medicaid Services State Operations Manual Appendices.
Informal dispute resolutions:
A Licensee may in writing request to dispute one or more deficient practices in the
Statement of Deficiencies (SOD) issued for State surveys or complaint investigations.
Possible resolutions to this request may be: 1) upholding the deficient practice,
2) modify the deficient practice, 3) remove the deficient practice.
Plans of Correction (POC):
The Licensee/Provider/Supplier's plan of correction is created and submitted to the
Office of Medical Facilities Licensing within 14 calendar days of receiving
a Statement of Deficiencies issued following a survey activity. The plan of
correction must include how the Licensee/Provider/Supplier has corrected the deficient
practice cited, when the deficient practice will be corrected, and how the facility
will monitor the plan to assure it is working and compliance is sustained.

The Office of Medical Facilities Licensing maintains licensing files available for
public review. These public files include applications, licenses, Statements of
Deficiencies, and Plans of Correction. These files may be reviewed during normal
