Hearing Aid Dispensers, Audiologists and Speech-Language
Pathologists Provider Type Definitions
Audiologists:
A person licensed to practice A
udiology
. (The identification, evaluation, and rehabilitation of disorders of hearing and related communication impairments)
Hearing Aid Dispenser:
A person licensed to perform the fitting, sale and servicing of hearing aids and assistive listening devices. (Including the making of impressions for ear molds and the measurement of human hearing solely for selecting or adapting hearing aids)
Dispensing Audiologist:
An audiologist licensed to practice both audiology and to fit and dispense hearing aids.
Speech-Language Pathologist:
A person licensed to perform the identification, evaluation, and rehabilitation of disorders of speech and language and related communication impairments.
Speech-Language Pathologist Limited:
A speech-language pathologist whose practice is limited to providing services to school children.
Temporary License/Regular License:
A temporary license is good for only a specified period of time, while the temporary licensee works under sponsorship of a person licensed by the Arizona Department of Health Services in order to complete the requirements for a regular license. Temporary license sponsorship arrangements must be approved in advance by the Arizona Department of Health Services.
